Continuum of Care Definition in Addiction Treatment

In a continuum of care, patients usually begin at the highest level of treatment they need and then “step down” to less intensive levels of care as they recover. In this model, our team of specialists personalizes and adjusts the individual’s treatment program to meet their needs both throughout treatment and after they’ve left the facility. 

It can be compared to a situation in which a person goes to the hospital for a heart attack and then is released without any further instructions for aftercare. Most people would consider this a dangerous oversight and a recipe for disaster. So many things could go wrong when that person goes home without medical direction. Despite this mentality, many people with substance use disorders receive the same kind of attention: temporary treatment with no long-term plan.  

Offering a continuum of care and various levels of drug addiction treatment can bridge care at every stage, from medically monitored detox to aftercare. At our facility, patients undergo numerous follow-ups and are constantly monitored to ensure their treatment plans are lining up with their needs and progress.   

Our Addiction Treatment Levels of Care

Everyone is different, and because substance abuse disorders can vary widely in severity, our drug rehab in Philadelphia offers different levels of addiction treatment, each designed to adhere to clients’ needs and meet them wherever they are in their addiction and recovery. Each addiction level of care provides comprehensive treatment and is designed with long-term sobriety in mind. 

Partial Hospitalization Program (PHP) 

The most intense of our drug rehabilitation programs, our Philadelphia partial hospitalization program (PHP), is designed to provide patients with daily care and support. Unlike residential or inpatient care, PHP takes place in a non-residential setting, meaning clients don’t have to live on-site as they go through treatment. PHP is usually a step down from residential care, as it allows clients to spread their wings a bit and begin their transition into sobriety.   

Partial hospitalization programs usually consist of 20 or more hours per week of therapy and are designed to help patients with complex substance use disorders and co-occurring disorders who don’t necessarily require inpatient care. PHPs may also be referred to as day programs, as they normally last up to 6 hours a day.  

During their time in this rehab program, patients will participate in daily treatment therapies, including an extensive therapy catalog of the proven, evidence-based curriculum. Outside of programming, patients will live in an approved sober living facility in the area. Our facility offers typical PHP as well as PHP for veterans. 

Intensive Outpatient Program (IOP) 

An intensive outpatient program (IOP) is similar to a PHP in that it’s a day program that doesn’t require clients to live at our facility. IOP is a step down from PHP and therefore requires patients to come in for fewer sessions. While we in no way leave our patients without daily access to our services, this program is designed to help them transition to a sober lifestyle outside of rehab.  

IOP sessions are often less than 20 hours per week, unlike PHPs. Patients may be able to begin their care at the IOP level if they’ve already completed detox and previous programming. However, it’s common for patients to go through each level to ensure they never feel as if they aren’t getting sufficient support or guidance.  

Our intensive outpatient program helps patients transition back to life outside of rehab. This level of addiction care still involves regular programming but helps patients transition to their normal day-to-day schedule. This program also allows patients to address outside triggers as they occur. Like our PHP, we also offer a specialized IOP for veterans. 

Outpatient Program 

Our outpatient program is our least intensive form of care. This program offers flexible scheduling designed to accommodate patients’ schedules as they continue to get back to their normal routines. A major part of learning how to sustain a sober lifestyle is getting back into a routine, which may include working, going to school, and keeping up with responsibilities at home. 

Transitioning to life after rehab can be stressful for many clients. Some are afraid of relapsing or feel as if they need more time in rehab. This program is an additional step that acts as a cushion to ensure clients remain connected to our specialists.  

Stabilization Program  

For some people, relapse is a part of the recovery journey. To help, we have a relapse recovery program that steers people back on track in their sobriety journey. The point of this program is to get patients back into treatment before a one-time relapse turns into a full-blown addiction. Our stabilization program services include counseling and sometimes readmission into a particular treatment program, depending on the person’s condition following their relapse.
